Delivery and Acceptance Receipt

All information on the delivery and
acceptance receipt should correspond with the information on the lease
agreement.

The delivery and acceptance receipt
should be signed by the same person who signed the lease, by one of
the personal guarantors, or by someone explicitly authorized to accept
the equipment on behalf of the company.

The delivery and acceptance receipt must
include the date the equipment was accepted. The delivery and
acceptance date must never be prior to the lease date.
